There is a part 1 and part 2 to my review. End result is excellent service and recovery. Yes, I would positively recommend to anyone to choose this specific location as a place for Rehabilitation, Recovery, and Extended residence for elderly. I had the opportunity to sit and observe the daily routines of the staff as well as the interaction and recreational activities of the elderly and staff. Great work guys/gals and best wishes on the success of your facility. Thanks again, This was my initial review, (perhaps more reactive emotional) My mother arrived Monday 07/23/2024. For recovery and rehabilitation for knee replacement. The level of interest and concern seems minimal based on response time and lack of interaction with staff. They did not have her medications requirements. Its been over 12 hours since arrival and she is in severe pain, and they still dont have her medications or same service as the hospital. She spent the night in one diaper and it was saturated because no one changed it... Hopefully it improves. Orland Update on my review, I would like to make positive input to my initial response. Sagebrook cares deeply about the service and providing a great patient experience. I have to the prompt response on my concerns within 1 hour of my report was initiated from Home Director all the way down to the direct CNAs... They expressed a great level of urgency to rectify anything that was not representative of what Sagebrook prides its service on. My mother has been doing very good every since. As well the Leadership, also prompted an in person meeting to discuss my mothers daily support and if their were any concerns that needs to be addressed. Rarely, does this type of interest and/or concern in nursing homes happen. And I can speak from experience working as an IT Technician across multiple Nursing Facilities as well as family members being in other Facilities like this in the Austin area. Sagebrook is definitely worth the time and service for anyone to allow their family member to spend time whether is long term, short term, or rehabilitation. We were very pleased with the care my great aunt received. Overall great staff. The admissions director, Lauren, was very welcoming and made me feel confident in the care she would receive. Her therapy went really well and she walked out of there pain free!

As a hospice provider, we have a lot of options to choose from. Sagebrook is 1 of our favorites to refer patients to and work with! Travis (social worker) is amazing at supporting the patients and families! Bonnie, Scarlett, Kelsey, Jess, and Chris just to name a few are ALL such a pleasure to work with! Keep up the great work!!

We appreciate the respect with which our parents have been treated (even though they can both bee a pain to deal with at times). Any concerns on our end or the staff concerns, we were all immediately notified. Moreover, your assistance in transporting our parents to a doctors office has been invaluable. As we lack a wheelchair-accessible vehicle, your support has eased our burden significantly. This place is a nightmare!!! I wouldn t take my dog here. I am appalled by the staff and lack of respect, disregard of care to the patients and the health hazards this place has. Mom was transferred here from hospital and placed in a dirty blood covered bed with feces not her own. The room filthy and infested with cockroaches, bug larva and beyond gross. Bathroom filthy and staff causing bodily harm to mom by ripping her skin off by harsh care and leaving bruises. Not enough room to tell how awful this place is. Patients stacked in halls wailing, urinating on floors and dinner hall invested with mold, filth, broken ice machine w community ice chests w dirty cups and don t look under sink or in drawers. You will find bug eggs, bugs? Mold, dirty gloves, and unknown liquid in filthy plastic bags. This place is a healthy and safety hazard and will be reported.

I would like to thank Sagebrook for the excellent care of my mother. My mother came into this facility not able to walk at all and she left with a big smile and is now walking. I cannot reiterate enough how grateful me, and my family are for the staffs performance for making this possible. Joyce for taking the time to introduce herself, by personally coming to my mothers room on her first day. Her warmth and demeanor made my mother very comfortable. Sandra on point professional and quick in taking care of my mother! This was a plus. Florence who took care of mother for a short time on the 200 hall. She went beyond on getting my mother checked in properly. Laura who made it all happened. She helped me get my mom admitted. Marty taking the time out to go over my moms medicine making sure she had everything that she needed. Martys patience in keeping me informed on her labs and any changes in her medicines. Evie, going beyond and making sure my mom was groomed and her hair always done nicely. My mom being African American, made it difficult for just everyone knowing how to maintain her hair. I applaud Evie for taking the time to make sure my mom felt good about herself inside and out. And most of all, I would like to thank all of the therapist in assisting my mom every day, with strategic exercises to strengthen her legs. Because of this, she is walking again. Travis for taking the time to inform me on all necessary information needed concerning my moms insurance to get the care she needed. I cant say that Sagebrook is significantly worse than most nursing homes, but I certainly wouldnt put my loved one here. I am a health care professional who provides services to patients in various facilities, so I have some perspective on the matter. Ive also been doing this for several years. Many of the nurses here are a disgrace to the nursing profession, from their appearance (street clothes), to demeanor (gross disinterest), to overall incompetence (they know little to nothing about their patients). Not all of the nurses are like that, but too many of them are. Trust me: there are better nursing homes in the area.

I was ignored when I first arrived, and finally led into a large dark room with big white scars on the walls from things that had been torn off. The food was inedible. During the night, the orderlies were tucking it up very loudly. I went out to ask if they could lower their tone, the male responded (loudly) this is my lowest tone, and you go back to bed and back to sleep! I decided right then that I was out of there. The next morning I called a friend to come pick me up. She arrived about 10 am. It was 3:30 pm when we decided to leave. A nurse came running after us, and all of a sudden my discharge papers were ready. When I got the EOB from my insurance company, they had charged them for 3 days. I wasn t even there for 24 hours. Needless to say I reported them to my insurance company who was pursuing the matter with the National Board. The worst living memory of my life.

Terrible place. They scream and berate my loved one for soiling her diaper. No consistency with the actual rehab they are supposed to be doing. She goes for 5-7 days with no treatment from PT. They do not follow orders from admitting hospitals. This is truly more like a hospice but the staff is not nearly as nice or compassionate. They are actually mean spirited. Don t ask me to call and speak to you about this. Simply retrain or fire your staff and get people who are compassionate and want to work.

My father s health had been slowly declining and he had a bad fall at home. The hospital had us make a quick decision on a skilled nursing facility. We toured a few places but had a good feeling about Sagebrook. Therapy was wonderful, the was food was decent, and the staff were kind and caring. He stayed for 2 and a half weeks and got strong enough to return home. We are thankful for our experience and care while we were there.

First impressions are important. I cannot thank Christine enough for encouraging( yes, she told me that we need to find another facility) us to find another facility for my loved ones post hospital stay. We found a fabulous facility with wonderful staff who understands how insurance works.The facility my loved one is using now did not insist on payment upfront because they know how the insurance process works( our insurance covers 100% after a small deductible which we paid.) It is also important to know what type of care the person will be needing while at the facility and also to know what type of insurance that person has. Even after reading his clinicals, she did not know what he was coming in for and mis-stated our insurance. I consider this to be a bullet dodged.

My wife transferred in mid-December of 2022 from Seton NW Hospital to Sagebrook Rehab and Nursing, a skilled nursing facility. First, Id like to praise the Physical and Occupation Therapy team, Anne Walsh and Colin Ferguson. They were excellent, and were able to motivate the unmotivatable. Good progress was made, and my wife went from not being able to roll over in bed to walking with a walker. Nursing care was a different story. There were a few bright spots, and Id like to praise them: Sheila Johnston RN, and aides named Tish, Netty and Aleisha. I may be forgetting one or two. The rest of the story isnt good. Ill be a bit graphic below, so stop here if such things bother you. I spent 6 to 9 hours with my wife each day, so I saw quite a bit. My wife has a condition that requires a low-sodium diet. I had to request that 5 or 6 times before they got it in their system and began to honor it. My wife arrived without bedsores and developed them while there. I pointed them out to staff, but nothing was done. I eventually treated them myself. She apparently contracted COVID-19 while there. She tested positive, but never developed symptoms. It could have been a false positive, but they didnt retest, and she was quarantined for two weeks. She had been in the hospital with colitis, so arrived with diarrhea, requiring regular cleaning to avoid what Ill call diaper rash. Some of the staff (not the ones named above) did not want to come to change her diaper and clean her up except on their regular rounds, which were every two hours on day shift and every four hours at night. She thus had to lie in the mess for hours at times. During times when I was there I would either request additional clean-ups, or just do it myself if there was no response. Since the diarrhea continued, in early January the staff became concerned that she might have C. diff, which is highly contagious and antibiotic-resistant, and potentially fatal. By the time she left theyd attempted to test her three times, with no results. Once she was home we reconnected with her primary care physician, who immediately gave me a kit to get a sample and have it tested. The results were positive. By this time my wife was declining rapidly due to the increasing C. diff diarrhea resulting in dehydration, and we went back to the ER. Since she was tested for C. diff by the hospital and found negative right before going to Sagebrook, I have to assume that she caught it at Sagebrook, but was not treated there. C. diff diarrhea is apparently rather acidic, so one of the effects of not cleaning her often was the development of diaper rash. In addition, unlike the hospital, Sagebrook uses scented wipes for clean-up. My wifes skin is very reactive to scents, such that she cant wear perfume without breaking out. Once I realize that the wipes were adding to the rash problem I brought in my own unscented wipes and asked that they be used. Sagebrook staff readily agreed, but the next day when I came to visit theyd be using their own wipes again. By the time my wife left Sagebrook her rear was excruciatingly painful and the skin was breaking down. I believe the problems largely came from two sources: Poor communication and the constant use of agency staff not on Sagebrook payroll. The use of agency people is unavoidable in todays healthcare, but it does mean that many days my wife was cared for by people whod not seen her before and didnt know special needs (like the wipes). Some of the agency people were excellent (Sheila, mentioned above, for example), but some were just showing up for a paycheck. The communication problem should be avoidable. It became apparent that the care team each day was not being told of any special needs (unscented wipes or low-sodium diet, as examples). The bottom line for Sagebrook is that the therapy team was outstanding, but the nursing care was lacking in many ways. If my wifes current hospitalization results in a need for skilled nursing care, she will NOT be going back to Sagebrook.

Cannot say enough nice things about Sagebrook. My husband (Tom Reid) was a patient there for five months until his death. The staff was always friendly and seemed to have as much concern for me as they did for my husband. He was well taken care of. I would recommend Sagebrook to anyone. Thanks.

This was my 2nd time to stay at Sagebrook. Both times I was treated with the upmost care and respect from ALL the people there! I would highly recommend this facility to anyone! Sandra, you will forever be in my ! You give your heart and soul to your job and it shows! Celeste, (i think was her name, shes the charge nurse) you are an angel! You made me feel so good and welcomed. And the wonderful lady that always gave me my meds, youre an angel too! (Im still taking the pink liquid that tastes soooo good!) My therapists, Paul and Anna, you two are awesome! I learned so much from you guys! I appreciate you taking care of me and helping me to achieve my goals and MORE!! Thank You both! We could not be happier with Sagebrook Nursing Home and Rehabilitation. Their care and dedication to the people in their care is phenomenal. All staff is so attentive. They work hard to engage with all in their care. No staff member walks past a resident without making eye contact and saying something positive. They treat everyone with dignity and respect. We have seen a remarkable improvement in Mom s cognition. She is happy, secure and content. She loves the food. They make sure all of her needs are met. She went to Sagebrook for rehab after a fall and in no time we recognized the difference in care. You never know until you know what care should be! We decided to make her a permanent resident 6 months ago and have not regretted it. The business Office has gone above and beyond to help us navigate elder care and resources available. The entire staff at Sagebrook was s to be commended for their commitment and loving treatment.

My great grandmother came here after a stroke for rehab in 2013. On her last day alive we came to visit her and she was complaining of a massive headache. We asked a nurse for a doctor as we were afraid she might have a blood clot or something in her head. The nurse said no doctor was available and it basically wasnt a concern as she was on anti clotting meds. Well later that night she died of a massive aneurysm from a clot in her brain. It was told to us that the only symptom that could have alerted people to what was about to happen was a headache. She was getting better and frankly these people killed her All because no one listened to me or my families concerns. A headache isnt always a headache. Dont send your loved ones here we didnt have a choice due to money and we regret it everyday.

The Sagebrook staff has been exceptional for my mother (and our family) while she was in rehab after a hospital stay. The administrative staff has gone out of their way to make the details easy. The medical staff provides generous and compassionate care and assessments. And the social workers and medical staff were outstanding in assisting the family with empathetically guiding our parents to the next chapter for them in assisted living. We all give Sagebrook our unconditional recommendation.

I am writing this review to share my wonderful experience with Sagebrook Nursing and Rehabilitation. My mother was in the ICU for a serious heart issue and was transferred to Sagebrook after she was discharged to help her regain her strength after 2 weeks in the hospital. My mom is also battling Alzheimers in addition to the heart issue she experienced, so I worried that it would be difficult finding a facility that would be able to give her the full, quality care she needed. I was so relieved to find that every single staff member we encountered was friendly, professional, and gave the highest level of care and attention to her. I especially would like to recognize the following people who worked closely with my mom: Verna Regier (administrator), Travis Kalina (social worker), Rene Taherzadeh (admissions coordinator), Paul Acosta (PTA), Allison Abendroth (Director of Rehab), and Anne Walsh (COTA). Thank you everyone at Sagebrook for taking such great care of my mom and for helping put my mind and nerves at ease during such a stressful time!

For the second night in a row I caught the night nurse stealing my Hydromorphone (Dilaudid) trying to pass off Tramadol for the Dilaudid when I called her out about it she took the Tramadol with her then tried to play it off that they were together on the med cart and she just accidentally grabbed the Tramadol! Tonight she tried it again I again called her out about it, but this time she tried to be adamant that it was so I spit it up and I said where was the number 4 that is etched on one side and she tried to say that it was melted off cause it had been in my mouth (2)inutes, I pointed out that the score iwas still there and how could the number 4 be gone but the score still be there, I told her that I was getting ripped off and that I was reporting it to the Administrator and the State and the DEA, and I was going to have it tested and when I told her that I was reporting it and having it tested miraculously she was able to give me a Dilaudid that I watched being removed from the blister pack which would be highly illegal if she had truthfully given me a Dilaudid from that same blister pack like she claimed. I then told her I was going to have the pill tested and low a behold when I went to the bathroom she claimed that she destroyed it. (again illegal)! Never go to Sagebrook in Cedar Park, their black night Nurses are junkie theivv Theivess. Im still reporting them!

Nursing facility may be good but Im reviewiing here the rehab. Terrible organizaton and no spirit of rehabilitation for highly functional individuals. Despite my eforts for them to rehabilitate so my Dad could use the walker, they discharged him wheelchair bound knowing he was going to a house with 2 floors and steps. He had plenty of insurance coverage to continue recovery but convinced him he was ready to leave without regards of where he was going. I feel there was not rehab mentality, just nursing home mentality for non-functional individuals. If you are going to call yourself rehab facility, then do what is needed to rehab the individual. I can attest the nurses and aides were very kind and helpful, but doctor and staff had more business motives than the well being of the person.

Mother in law went here for a temporary stay after constantly falling. While I can t speak for the therapy staff her nurses were annoying. They would constantly tell her she needed to go to sleep and quit staying up so late all the time..... apparently 10pm is late. One night she had to get up because a nurse left the room at 11 pm and left all the lights on in the room. Specifically the one in her room mates face. When the neighbor said she needed the light off, my mil went to turn it off. The man walked back into the room and yelled at my mother in law to go to bed! She said that the way that most of the people treated her was like she was a child and shouldn t do anything without permission. The evening staff yelled at her every single night. When we went to visit at different times during the day everyone seemed to be unhappy. The patient and the staff. When we asked a staff member questions they seemed we were annoying.... something has small as if we can bring outside food into the patients. (Answers yes by the way) When I called to talk to the director of nursing, the receptionist was rude when spoken to. She put me on a 22 minute 4 second hold, came back and said she would transfer me. Then I got a voicemail. Overall therapist might be good (I haven t heard a complaint about them from mil yet) but everything else I ve heard and seen have been awful.

We have a close friend in this facility for rehab...she had been there for 3 days when we went to visit her due to her calling with request to be moved. She hasnt been showered in three days (WE GAVE HER SPONGE BATH CNA GAVE US TOWELS RAGS SOAP)..no therapy which her Dr ordered that she walk every hour.. She had no clothes or shoes the family had taken them from her along with her phone. While we were there my other friend had asked that the social worker send a REFFERAL to another rehab for an assessment (closer to her home)..instead she called the family and the S.I.L walked into her room and began screaming at her to LAY DOWN THEN HE PROCEEDED TO TELL US TO LEAVE OR HE WAS GOING TO CALL POLICE 2 NURSES CAME IN AND THE PATIENT STATED THAT SHE WANTED TO BE MOVED AWAY FROM THIS FACILITY IMMIEDATELY..THE DAUGHTER SHOWED UP WITH THE SOCIAL WORKER AND TOLD US THAT WE HAD TO LEAVE AND NOT RETURN OR WE WOULD BE ARRESTED!! THAT WE WERE INTERFERING WITH HER MEDICAL TREATMENT AND IT WAS AGAINST POLICY FOR US TO GIVE HER A SPONGE BATH !! I HAVE NEVER BEEN TREATED WITH SUCH DISRESPECT AND MY POOR FRIEND BEING TREATED SO HARSHLY IN FRONT OF THE WHOLE FACILITY. THE WHOLE EXPEIRENCE IS VERY DISTURBING AND ITS CRUEL TO ALLOW A PERSON TO BE TREATED THIS WAY.

My mother was at Sagebrook Health Center for a little over three years. They were wonderful to her and very caring. Like all nursing home situations being very involved and being there frequently is important to your family member and for the nursing home. They showed to be honestly caring and loving to my mother. The people in the office were invaluable assets in my advocacy to my mother s care. I saw repeatedly care takers loving on the patients.

My mother-in-law passed away 6 months ago and Sagebrook continues to hound us for and old debt. It has been made clear in writing to them on multiple occasions that she passed away with no estate or assets. There repeat reminders are hurtful to my wife and we will be taking legal action if they continue.

DO NOT EVER SEND A LOVED ONE THERE! The staff are mean and treats my friend terrible and she says its hard to get anyone to give her a shower. The food is awful and the night shift is loud where she cant sleep.

My brother was here for Rehabilitation for one month. The staff was wonderful and caring. He got occupational, speech and physical therapy. There were no thefts. I would highly recommend this place . He did need a patient sitter for a couple of weeks as he was disoriented and we used Companion Care Angels.

My loved one was in sagebrook for short term rehab, and the therapy department went above and beyond. He is at home and thriving now. Thanks to the great nursing team for getting him back on his feet!

PLEASE PASS THIS PLACE UP! My grandmother stayed here for a while after the hospital. In the mornings when family would go in to visit, my grandmother would be so uncomfortable and upset over her treatment and lack of care. Her bandages would be coming off of her OPEN wounds and it took a couple hours for them to get around to bandage it. My grandmother said she would buzz for help over and over throughout the night and the nurses never would come, she said she feared she made them mad and they didnt want to help her anymore. That is your biggest fear when leaving a loved one in the temporary care of a facility! There was one nurse who was helpful, but she told us she had too many patients to take care of before getting to my grandma. Sounds like they are understaffed and dont have enough people to provide proper care for others.

After having a bad stroke my father was in Sagebrooks rehab wing. The therapist were great, but the other parts of the faculty are very poorly run. The nurses were awful. My father was left in soiled clothes for hours and the food looked awful. Do not put your loved one here.

Not the most beautiful facility in the world, but the people there more than make up for it. My father was recently ill and in rehab/nursing care at several places, and by far and away Sagebrook was in every way superior. From the moment I first walked in the door I could tell it was different: all the employees I saw were busy and involved, and very ready to help with a smile.

I would like to take this time to say THANK YOU SO MUCH to Sagebrook Health Center for making the last few months of my dear Mothers life as enjoyable as possible. I moved my Mother to Sagebrook on October 5th, 2012 and she was immediately at home there. Her RNs, CNAs and everyone else involved with caring for her were so good to her and I appreciate it more than words can say. She had her favorites but it was clear to me that she was the favorite among most of the staff. I want to thank Sarah for recommending such an excellent hospice service because they came in and went straight to work making Moms final days as comfortable as possible. Mom passed away with dignity on Saturday January 26th, 2013 and I will miss her for the rest of my life. Thank you Sagebrook for taking such wonderful, loving care of the dearest Mother anyone can ask for.
