This place has been a blessing I had my dad moved from a facility that I was not happy with the nurses and the administration has been nothing but amazing with helping me navigate thru this difficult time Heather and Brandy made sure my dad was transferred in a timely manner and my dads nurse Joel has been so helpful even the maintenance guy was wonderful and went above and beyond to help me get my dad comfortable I would highly recommend!!!

Despite all the negative comments about staffing I worked at this place a few times with agency. Yes people we are in the times of nursing shortages. We do our best for each and every person when we can spare a minute or so. That s why during the weekdays there is extra staff due to management being there to assist residents and their families. Please don t down the nursing staff. You never know when it may your turn to have the entire staff in your room performing CPR on you to save your life or your family members life. We (nursing) are exhausted, tired of being shamed and told to do better even though we are doing our very best. We pick up extra shifts to help with the shortage to assist the facility in saving money on not using agency. I hope this review helps those in need of reading it. Instead of complaining about how something wasn t done why don t you show your appreciation for what did get done. Now that s a hard bullet to bite. I promise those when you focus on the small things you tend to be more helpful during the big things.

Do not take any of your family members to this place unless you are willing to risk their life. I made this mistake and my father died because of it. Many of the staff members just dont care about the residents. Too many mistakes and just plain incompetence on behalf of the staff that works there. These types of nursing homes should not exist without the right staff. Beware.

The employees that I have had the pleasure of being around are kind, polite, friendly and caring. The facility appears to be older but has a very home like feeling. I love the fact that there is an activities director and department, they actually do activities with the residents and the families are invited to participate. I also love the fact that they are trying to work with my uncle on physical and speech therapy

I had my father transferred here recently to be closer to family. He always seems very happy, and the staff is very courteous. They did a such a wonderful job during the COVID pandemic letting the family have window visits and Zoom calls.

Word of advice, DO NOT take your relatives to this facility. I agree with a previous comment, the place should be shut down. The staff is very unprofessional and never looks at patient charts to see if they have special needs such as a specific diet, breathing treatments or appropriate physical therapy. They can careless if the patient gets better or dies. As long as they are getting paid.

TERRIBLE PLZ DONT TAKE YOUR RELATIVE HERE. On weekends people just dont care.A lady needed help urgent, and the ladies took their time to walk slowly instead of running and reacting fast. I used to be at this facility for 8 months whave been trying to return for personal reasons and even though Ive won my appeal and facility was ordered by state to allow my return, the administration has still refused state order, slandered my good name and blackballed me with doctors who work for facility, as well as other facilities in area. I only want to be therefor a short time to use as a stepping stone to go tan assisted living facility and to be close to my family. Seriously how much hatred does it take to keep me from that, and disobey a direct order from the state. All I can say is wow. Been trying to resolve this nonsense for months now. Beware of good reviews from local guides who are paid to promote facility, or workers who are in key positions, trying to make their facility look the best. Every facility has its good points and bad points. Talk to tenants or residents without workers around in order to find out how a facility is, or go visit it after hours when administrators go home is another good way to find out. Hope this has been helpful to anyone who is looking for nursing homes and rehab.

This place is terrible! Im trying to get my Dad out of there now. Do NOT let anyone you care about live or stay here. All the people working there are mean to the residents. They never get to go outside, the rooms are so unsanitary and have nothing in them, they keep it very hot. All the residents are basically given heavy tranquilizers and left in their beds sad and confused. This place should be shut down. Period.

**PLEASE STAY AWAY** PLEASE PLEASE! this took me a while to write because i am not one to EVER write or talk negatively about a community but THIS PLACE NEEDS TO BE SHUT DOWN! my father was here in the beginning months of 2018 and that is a REGRET ill forever hold. i chose this place after knowing there was only one other rehab place here in grand prairie and this LOOKED better and the tour i had with management was better than the other place i looked at. my father wanted to stay in grand prairie as it was close to his dialysis center. per his wishes i stayed in grand prairie and chose this place for him, AGAIN, i will FOREVER REGRET. first, the NURSING STAFF IS ABSOLUTELY UNPROFESSIONAL AND NOT RELIABLE! one night my brother and mother call me saying i need to get to this place ASAP, when i arrived my dad was not responding! he was breathing but verbally not responding with eyes closed, i would say his name and he would just raise his eyebrows. i get the nurse and speak with her and THE FIRST THING SHE SAYS TO ME IS IVE BEEN OFF FOR A WEEK SO I DONT KNOW TOO MUCH ABOUT YOUR FATHER. first off, i too am a nurse and the first thing you do before your shift is GET REPORT from the previous nurse. at that time YOU SHOULD BE GETTING ENOUGH INFO TO BE ABLE TO PROPERLY, I REPEAT, PROPERLY TAKE CARE OF THE PATIENT. FOR GOODNESS SAKE SHE COULDNT EVEN TELL ME WHEN THE LAST TIME MY DAD HAS HIS NARCOTIC PILL!!! and she kept repeating shes been on vacation. i then tell her to take a look at my dad, shes comes into the room and calls my dads name out and again my dad just raises his eyebrows, she then states well i cant send him to the hospital BECAUSE HE IS ABLE TO RESPOND BACK. are you kidding me? thats a response? she didnt even take his vitals until I ASKED HER TO! i then tell her i want him out of here and to the hospital because I HAVE NEVER EVER SEEN MY DAD THIS WAY, EVEN WITH ALL THE MEDICATIONS HE HAD FROM HIS AMPUTATION (the reason why he was sent to rehab). my dad was NOT RESPONDING. i had her take my dads vital and her vitals were normal so then says oh theres no reason to take him his bp is fine. ok...but LOOK AT HIM! i mean she didnt even want to try to see what was making him this way, she just took his vitals and pretty much said oh hes fine. really YOUR PATIENT is NOT RESPONDING OR OPENING HIS EYES NO MATTER HOW HARD YOU SHAKE HIM. FINALLY, THE PARAMEDICS CAME AFTER I HATE TO TELL THE NURSE THREE TIMES, THREE TIMES, TO CALL 911. WHEN THE EMS CAME, THE CHARGE NURSE COMES IN AND TRIES TO FIGURE OUT WHY THEYRE TAKING HIM OUT TO HOSPITAL (AGAIN POOR COMMUNICATION, THE CHARGE NURSE SHOULD HAVE BEEN NOTIFIED BEFORE IT GOT ESCALATED TO THIS POINT) THE CHARGE NURSE WAS MORE CONCERNED WITH A PATIENT LEAVING HER COMMUNITY AND MAY NOT RETURN THAN THE ACTUAL HEALTH OF HER PATIENT. i shouldve listened to my instincts. THIS PLACE ALSO HAD A REALLY BAD CALL LIGHT RESPONSE TIME ISSUE. WHEN I WAS THERE AT NIGHT THERE WERE TWO TIMES WHERE I HAD TO GET THE NURSE FROM THE NURSE STATION WHO WAS JUST ON HER PHONE TO COME HELP MY DAD. AFTER ALL THIS MY DAD WENT TO ARLINGTON MEMORIAL AND STAYED AT KINDRED ARLINGTON WHOM I ABSOLUTELY RECOMMEND AS THEY ARE AMAZING! my father passed in july and one of my wishes is to have never placed him here. THE STAFF DOES NOT DOCUMENT MED ADMINISTRATIONS OR IS LEFT UNORGANIZED. I MEAN THEY COULDNT EVEN TELL ME WHEN THE LAST TIME HE RECEIVED HIS MEDS AND THEY ARE OBVIOUSLY NOT DOING 2 HOUR CHECKS OR ELSE THEY WOULDVE CONTACTED US ABOUT MY FATHERS CONDITION BEFORE MY FAMILY GOT THERE AND HAD TO TELL ME THEMSELVES. THE DON WAS THE ONLY PERSON TO CALL ME AFTER MY FATHER LEFT THEIR BUILDING TO FOLLOW UP ON WHAT HAPPENED. THE ADMINISTRATOR CONTACTED MY MOTHER AND MY MOTHER INFORMED HIM THAT ITS BEST HE SPEAK WITH ME AND TO GIVE ME A CALL, WELL HE NEVER DID. again, I SHOULD HAVE CALLED STATE, but i was too busy taking care of my dad. I HOPE THIS PLACE GETS SHUT DOWN, THINK ABOUT OTHER PATIENTS THERE THAT DONT HAVE FAMILIES DOTING ON THEM. MY ENDING STATEMENT IS **PLEASE DO NOT PLACE ANYONE HERE**

I stayed there for 3 weeks on their 100 hall (Rehab). I was very sick with a bone infection when I was transferred from the hospital. My hip was so infected I could barely move my leg which meant I had to wear diapers and be changed in bed. With the help from Rehab, Aides and Nurses, I got strong enough to be able to visit the bathroom on my own and eventually come home! The Rehab really encouraged me to work hard. The whole rehab therapists were extremely friendly and treated me as if I were someone special! (I was emotional when I was discharged because I knew I was going to miss their friendship.) Thank goodness for modern technology! Kuddos go to my Occupational therapist, Deepa Moses and my Physical therapist, Amber Plowden, and the other therapists which I hope I get their names correct... Amanda, Marsha, Alex, Amber, Sherika and Deepas colleague that sits next to her! Haha (You know who you are!)) for their encouragement and friendship. My main nurses Ruth, Josh, Joyce and the others were very friendly, good and helpful! As far as the food is concerned, could be improved in my opinion. I seldom received a night snack due to the lack of snacks sent from the kitchen and I was required to provide my own to ensure that I received one. Im a diabetic, so it was important to me. The kitchens idea of a sandwich OR burger was served dry without mayo or mustard on stale sandwich bread! The food trays stayed in the hall from 30-45 minutes, so no wonder the food was on the cool/cold side. There are other complaints but there are too many to mention. I just think the kitchen could do a better job in serving decent food at every meal. I visited with Wanda about which foods I liked/disliked and they did their best to comply. I must say I did enjoy the breakfast there every day. Like I said before, when it was served warm. I thought my hall had no bad smell to it, but there were some halls that really smelled like sour urine! I tried to shower in the two shower rooms but I couldnt take the strong urine smell! I thought the office staff and my social worker, Isabell were quite helpful in my discharge. Thank you for all your hard work and the many phone calls you made, girl!

Joel here..I am a resident here...this place is Wonderful.....its very clean...nurses are extremely knowledgeable. The review from BECKY is totally wrong... They have an excellent cleaning staff! Also there good is excellent and very nutritious.they have to follow strict guidelines set by the stste. I love this place!!

Do not take your loved one here! My mother used to get abused by nurses. If I could I d sue this facility over their lack of care & the cause that lead to my mothers death. This place only guarantees death... and she was only trying to recover from a stroke. She wasn t even of age. I repeat DO NOT EVEN TRY THIS PLACE JUST VETO.

The worst facility I have ever been to or heard about! Its filthy it smells the entire staff is rude and acts totally lost! They dont even have anyone at the front desk to make sure of who is coming and going, its about 7 employees sitting behind nursing station doing nothing! My mom was there two days and I could not get her out of there fast enough! They didnt give her her meds it was hot in her room walls were dirty and disgusting her mattress was so old it wouldnt lay flat! I will definitely be calling the state on them! I wouldnt trust them with my worst enemy! Do not put your love ones there!
