On behalf of my 80 yrs old multiple seizures dear hubby, I would like to express our family gratitude to Sally Coste, a compassionate RN who goes out of her way to properly feed my husband s meds and vitamins thru a NOT simple PEG in his tummy. She certainly is an asset to Baywind Village Skilled Nursing & Rehab Facility. And would highly recommend her to be your Employee of the month. Thank you Management for listening.

I wanted to take a moment to recognize the High Risk Assessment nurse Sharry. She embodies everything that Baywind stands for - compassion, attention to detail, dedicated to her patients and a great team player. She is thorough and handles any request given to her with a smile on her face. I know that when she is involved, everything will be handled with great care and followed closely. Thank you, Sharry, for your dedication to the patients!

The staff is for the most part very kind and caring... I wish I could list everyone that has been there for my mom these last couple of months but I was told I could only refer 2. Natalie and Briana have both really been crucial to my Moms recovery. Especially early on when there was so much uncertainty about everything, both of these ladies made life easier for all of us. We are so grateful to both of them and this facility is very lucky to have them.

My MIL was here for a few months with a broken femur. The physical therapists were great, in particular Coffey and Oly. They were always very friendly with a smile on their face and always very positive. You can see the passion that drives them to work with the patients and empathy in the way they treated my MIL, but at the same time pushed her to work hard to get up and moving. I appreciate their professionalism and passion with her. She cried when she came home because she was going to miss them. Overall the nurses, med techs and CNAs were awesome with a few exceptions that were reported to management. One of the main issues with the nurses is that her pain medication took hours to be given after she pushed the call button, one nurse in particular seemed bothered by her request. It was reported to management and they spoke to the nurse.

My mother broke her femur and we were faced with looking for a place where my mom can be taken care of. We chose Baywind because of their recent reviews so I would like to be very honest in my review for those who are looking for the best place. Jesse the shower technician was amazing with my mother, even when my mother would have accidents ( poop in shower) Jesse would joke to make my mother laugh and not feel embarrassed. Maria in housekeeping would come to her room and give my mother lots of words of encouragement. Maria and Jesse really connected with my mother. The CNAs were All exceptional from the young ones to the older ones, they were very professional and those are the ones I thought we would have issues but didn t. My mother had exceptional nurses but did have 3 nurses who were truly rude .the issues we had with them was administrating her pain meds . It would take more than 1hour 1/2 for the nurse to bring my mom her meds . Each time we asked a question they would make a face like we were bothering them. Even some CNAs would say they couldn t even ask them for help because these 3 nurses didn t like to be bothered. I saw that the people with the hardest jobs to do were the ones who went above and beyond with care and compassion. Did have to go to administration with the issues and 2 of them had a change of attitude like bringing her meds on time. The other one nurse continued to have an attitude . I don t want to name these nurses because everyone deserves to have new opportunities .my mother actually cried because she was going to miss Jesse and Maria .

My MIL was admitted to Baywind after a brief stay in the hospital to rehab back to being able to walk after she had finished chemo and radiation and became very weak and unable to walk. My first impression when bringing her to the facility was not great and my gut said don t leave her, but she insisted it would be okay. First off, we were not warned that they had Covid positive patients before arriving and my MIL s immune system is already shot because of chemo. They should have let us know before ever considering admitting her. Well, she got it and had to be quarantined for 10 days. The staff is not friendly or caring. When we arrived that just pointed us to the direction of her assigned room and said go. I felt like they didn t even want to be there and if we had any questions, which is our right to ask, they acted like it was an inconvenience to them. I would have to get someone above them. Candice is the head nurse and I left a VM on a Friday afternoon and still hadn t received a return call by the following Tuesday when I had to go to someone else to get her to call me. Then she played all nice and concerned. Through our phone conversation, they were supposed to update us with her PT every couple of days, which we NEVER received. Likely because they never did PT, which is why she left in worse shape than when she got there. I understand they cannot force a patient to do what they need, but if the patient isn t receiving the therapy and care they re admitted for, they need to discharge them so they can get to a facility that will give the right care. We took her out of that facility ourselves. Baywind would have billed the insurance until my MIL passed if we hadn t taken her from there. Besides all the shotty care she received, or we ll say didn t receive, the food they serve is god awful and I wouldn t feed to a stray dog. If we hadn t brought her food daily, she surely would have starved to death and was close to it. We complained daily and they said there was another option besides the standard menu, but they would rarely give her anything other than the standard menu. She stayed three weeks and the day we took her from that facility she was admitted to the hospital that night extremely malnourished and dehydrated. Her oxygen was at 27. She was near death. Now she is in home health care with us where we can monitor her therapy and food and hydration and is back to recovering. No way would I ever consider recommending this facility, unless that is where you send your loved one to die.

He has dementia and she was very understanding and patient with him and encouraged him to eat and she was very attentive in making sure he was not chewing too much or pocketing his food. She put him on a purreed diet while he built up his strength after having a severe UTI, aspiration pneumonia, and then Covid. He really needed to build up his strength again and Dr. Chinu made sure that he was doing just that.

I have soooooo many reviews Id like to give on this place....first let me say the place itself is nice, clean and has beautiful landscape thats very well kept. My Mom spent 20 days here for physical therapy to regain her strength. I was there everyday with her so I got to see and meet alot of the people there. MY ABSOLUTE 2 FAVORITE PHYSICAL THERAPY GUYS ARE Coffey and Marty!! These guys made my mom feel comfortable from the start of it all...they spoke to us both like they had known us for ever and that itself to me speaks volumes. Nearly all the aid ladies my mother had were just amazing, again making her feel comfortable about asking to be changed or if she needed something some were overly attentive and that was a huge plus for her. Jeanne and Sylvia were my moms FAVORITE LADIES, these 2 went above and beyond for her. I will admit there were some slight bumps along the way but the wonderful Staff made sure to take care of them right away. IF my mom ever has to be in a skilled nursing facility again we will definitely come back! Although its a joy being home, I miss all the friendly people. Thank you All!!

I have visited several long term care facilities with various family members and this one has blown me away! The cleanliness a d care of the staff is top notch. I ve never seen a crew of employees that care so much; you can tell they are treated well and want to be there. All staff we have been in contact with seem to be truly caring and continuously checking on everyone s well-being. I ve seen staff members stop what they re doing to help someone or go out of their way to do something for someone on numerous occasions. I wish all my family members could have stayed here. One example: Monica had cared for our family member recently. He was moved to another room out of her area - she has a connection with him and she s a magic-maker for the stubborn guy. He wasn t willing to eat and Monica just so happened to pass by and see him as she was walking down the hall, She checked in to see him, heard he wasn t eating. She had to finish her task she was doing at the moment and then came back and staid with him, talked and got him to eat. It wasn t her responsibility at all but it shows how much she cares about patient care and wellbeing. She even helped him with the restroom you know that s not a fun task but she did it with a smile to help him out. She really cares and just as easily could have called for the responsible tech or nurse s aid. I m so impressed with all the crew; they ve all showed compassion like this

Great morning! Here s my little storyette about an awesome experience I had at Baywind Village. My mother is a current resident at Baywind and it was time for renewal of all necessary contracts to be submitted. Covid had come and gone and some very important paperwork had slipped through the cracks. Well it wasn t until months down the road that this paperwork not filled out and submitted had turned into a code-red situation. Me and my wife both work full-time jobs and had not realized the time sensitive material was way past due. We had been granted grace due to many going through the same covid induced scenarios. Well, needless to say but this happened a few times and now it was all hands on deck to somehow rally the troops to turn a situation that could possibly be pending my mother s residency. Through many hours and delays we pressed through the crowds and finally made the torch relay! I was working with quite a few incredible ladies on staff at this facility but one in particular was Ashley Leal. She was going above and beyond what was required and expected of her. Through our combined consistency and communication we managed to get all paperwork submitted for necessary requirements so my mother s stay at Baywind could be extended. I love this place. I love the people that work here. I love their heart to serve. I love everything about it. And they continue to out do theirselves. It s a game changer when human beings step up to the plate and come through for a family as if it were their own. That s the difference right there! I know there are more stories out there just like mine. Hit the review button below to share yours!

She was very helpful and knowledgeable. She was most accommodating. I certainly appreciate her efforts and I have been making great progress since moving to my new room and I want to send a special thanks to Ashley Leal. This review was previously posted and deleted by mistake: The truth is no facility is perfect... All organizations, no matter the size, will sometimes have less than optimal situations. What is most important, in my opinion, is how an organization responds to these opportunity for improvements. Good organizations quickly address these types of situations. Great organizations do not stop after this step. They strive to learn and understand the underlying factors contributing to the situation. They implement any needed changes. Most importantly, they share their finding with the rest of the organization so everyone learns from the opportunity for improvement and is better prepared to prevent similar situations in the future. Baywind Village is a great organization! Great organizations do not just happen. They always have one or more people who actively drive improvement . I suspect Baywind Village may have a number of these people, but the one I have observed during my stay is SHELLY OSBORNE. Shelly is always looking for opportunities to make the Baywind Village experience better. She has certainly been successful doing that for me and other patients/residents during my stay. Thank you Shelly for all you do!

The nurses at Baywind Village are a fantastic group! However, no matter how good a group is, there are always high performers who stand out. My standouts are George, Wasan, Kris, and Sharry. George stays busy all the time. In fact, I ve never seen him sitting down. Wasan has a different approach I find very calming. Of course, she still runs a tight ship. Kris is so very kind and considerate. Then there is Sherry Sherry is so good at her job, it is difficult to explain. She has helped me through several procedures I find difficult and upsetting. The last time she did this was on her day off . That is dedication!!!

Always makes sure Im where I need to be, she does my wound care when the wound care nurses arent on duty. She changes my dressings and gives me my antibiotics on time. Always controls my pain and brings me coffee. Clarissa the cna does a great job giving me the best showers I only trust her. and brings me ice and keeps my arear tidy and changes my linens often. They give the best care and I can tell they love their profession. Thank you with all my heart. Vincent garza jr

I have come back to this facility for rehab. Although it has been a rough time with my health I would like to express 2 ladies that have made my days and have the brightest smile and such kindness that I look forward when they are on the 314 hallway. Lisa an awesome Med Tech offers her help above and beyond her job. If there is something I need she cheerfully takes the few mins to take care of it. And there is nobody like Maria in housecleaning. She cleans cheerfully and is concerned with your needs. Brings sunshine to my room. Both of these ladies I would hire on the spot if I needed someone to supervise my business. Both an incredible value add to Baywind Village They will be missed when I m done with rehab.

Unfortunately I had some health issues recently that required Physical Therapy. I asked around, did some research and learned Baywind Village had a strong Therapy team. All of the Therapists are good but one really stands out. Oliy Ferrier. She is totally dedicated to her craft. Oliy is Super knowledgeable, compassionate, and a real pusher. Everything one could ask for in a trainer/therapist. Shes all business, tough as nails with a heart of gold. Another stand out is Angelia Butler. If youre willing to work, those girls can get you where you need to be. I came to Baywind because of the therapy team but I found the rest of the facility to be top notch. All the staff I came in contact with were professional, friendly and helpful.

Shelly, The Director of Social Services goes above and beyond with her always cheery smile and visits with my mom. Nothing is too much for Shelly, her presence alone is mood changing. Nadja (sp) her nurse, Ashley, one of her aids, and basically all the 300 corridor are so good to her. Ashley her aid facetimes us when I can t make it, touches base w me via text, it means so very much. I m sorry I can t think of her other nurses name but a shout out of appreciation goes to her. DO NOT PUT YOUR LOVED ONES HERE! We chose this facility after my very able grandmother took a fall and fractured her pelvic bone needing rehabilitation. This place is horrible. I dont see how the nurses and aids can sleep at night knowing the care or less than that they are giving the patients here. In the hospital before going here my grandmother was working on getting up and learning to walk to the restroom with assistance. Once placed here she was immediately put in a diaper and NEVER helped to the restroom. She was here for 9 days before I finally demanded her to be transferred elsewhere. In the 9 days she had 1 shower! Another time she had pressed a call button and we set a timer it took someone 17 minutes to come into her room. The nurses are BEYOND rude. The only nurses I didnt have issues with were the head nurses Candice and Melissa when I went to them both for several of my complaints. I was walking in the hall to my grandmothers room once and a gentleman in a room next to hers waved me down in a panic as his roommate had fallen out of bed and no one was coming for the call button. I went and notified a nurse to which she said Oh my God not again and got up seemingly frustrated. I finally feel like I can breath now that I have transferred her from this facility. This is not a rehab. This is a facility full of beds you put your loved ones to die.

First impressions are everything. I have been practicing audiology for 15 years and during that time providing services and hearing care to many patients in facilities, skilled nursing, and assisted living. What I experienced in my short time at Baywind Village was excellence! The level of communication and teamwork that you can see just by walking down the hallway is so impressive from administration, maintenance and the service care providers they are extremely engaging with the residence. And it is clear that their priority is the residence experience. Big shout out to Shelly Osborne not only personable, friendly and knowledgeable, but provided me everything I needed to make sure those residence had the best experience possible with my services. Stayed with me every step of the way. In my opinion, those are work ethics that are rarely found in facilities that have so much going on. My brother was unfortunately only there for a week or so, but while he was there, he received the best are and attention to detail. The nurses were wonderful, and Shelly, the social worker, was amazing. She helped my brother, and made time to spend with me as well, helping me understand my brothers circumstances. She was very kind and understanding, and explained things in a plain, non-dramatic way that made them easy to understand and relate to. Nadja helped make the move less traumatic and was always available to answer questions. All of the nurses and aides were fantastic, both from the facility and from hospice. We were very worried about this because of all the negative press that usually surrounds skilled nursing and rehabilitation facilities. We could not have been more pleased with our experience. Starting with Ashley, the Admissions Director. She handled all our calls and smoothed the complex paperwork and admissions process. She reassured us and calmed our anxiety about the transfer. She could not have been more professional. The staff were very caring. They made our relative feel cared for and important. The skilled nursing staff was professional and knowledgeable even with complex wound care issues. The Physical Therapist and Occupational Therapists and the therapy were wonderful. He regained strength, stamina and confidence. He went from bed only to sitting, standing, and walking. He was able to negotiate hard transfers from bed to chair and back. We cannot say enough positives about the loving care provided by the Baywind Village facility and its staff. They strive to create a professional yet caring environment that is more like home so that each patient can grow to their full potential.

Although, they appear to be short handed at times, this facility was always clean and satisfactory. My mother was there from 5 months and received more than adequate care for being bedridden. She was always fed and cleaned. There are social events daily to keep the patients involved. The CNA, Lisa M., is by far the most compassionate nurse there. She goes above and beyond with the compassion and understanding she provides to the patients and their families.

The MDS nurses at this facility Mrs. Rose included have stolen Covid test that were for use on the patients and staff yet were taken home to use on neighbors and friends. After an internal review this was swept under the rug and not a person held accountable.Mrs. Rose who works as an LVN says she has to fill out all nursing notes done on patients because the they simply wont do their jobs what kind of place operates this way. Talking about in house patient care to family and friends outside of the facility is a clear violation of HIPAA laws. The toxic atmosphere Mrs. Rose talks about her coworkers and owner boss Chris should be a clear indication to never even think about the care given here . Mrs. Rose talks about nurses at night sleeping in their cars when they should be inside at work! The Doctors that call on this location either don t care or incompetent? This coming from someone who works inside this facility! Beware I d say for sure!! The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal law that required the creation of national standards to protect sensitive patient health information from being disclosed without the patients consent or knowledge.

We LOVE Baywind!!!! The office staff, Ashley Leal, is awse!! They went above and beyond in taking care of my mother! The Nursing staff and everyone that wworks on the floor are awesome! Everyone is friendly and show great concern and compassion. I would like to mention 2 people that took care of my mom while she was there. Alaya and Matt! These 2 stand out to me because I saw them the most. They took such good care of my mom and also would take the time to speak to me and other family members about my mom. I feel like the staff from the office to the nirses are a large group of people that truley care about taking care of the people at Baywind. If you ever need help taking care of a friend or loved one Baywind is the right choice! The therapy team is just as good, Angela, worked with my mom and me to get her where she needed to be and make sure I also was able to take care of my mom when she came home. A HUGE THANK YOU to all of Baywind and especially Ashley Leal , Matt, Alaya, and Angela my mom is doing great!

Our mother has been receiving rehab at Baywind for 6 days. I do want to praise one of her nurses Jeff ( navy Veteran). He has shone our mother compassion and was very prompt about calling the on call Dr. to get orders for a cxr, lab work when she was short of breath. I ll write another review after she spends a little more time there. I do wish Baywind could work on making it easier for families to get in touch with their loved one & staff. I would like to thank the DON Ramona for her quick response to all of my emails regarding my mothers care at Baywind village. And all though she is no longer receiving care at Baywind her favorite angel she called Matthew (Matt) will never be forgotten for his compassion and respect to me and my mother. I m writing a second review because i feel it is important to recognize a few people here at Baywind. First I would like to give a special thank you to Mona Lisa (day shift aide) who truly goes above and beyond for her patients. She became like family to us and we can t thank her enough for taking such good care of my father. Her kind spirit truly radiates and though my family and I are going through such a hard time she seemed to make those tough days better. I would also like to thank Barrie s Pettis (day shift aide), Jalin Jackson (day shift aide), Inga (night shift aide), Montrice (night shift aide). As these individuals spent much time with my father and truly provided the best care for him. My family and I can t thank you guys enough for everything y all do and all your hard work. I also would like to thank a few nurses Patty (day shift nurse), Stephanie (day shift nurse) who spent most of their time with my father and I couldn t thank them enough for always being there and making sure my father was well taken care. No matter what these ladies where always considerate and truly listened to our concerns and did the best they could for him. Also thank you to Brooks (night shift nurse) who helped with my fathers care and always had a bright and bubbly attitude. My mother and I never left my fathers side we where there everyday and night with him so we where able to observe everything and everyone. After everyone got use to us and how we wanted my fathers care everyone quickly adapted and where able to meet my fathers needs, as he is unable to communicate hisself, we had to advocate a lot of his needs for him. I would also love to thank the respiratory team, physical therapy team, and wound care nurse. These people especially physical therapy touched our hearts they really try their best to help my father improve and everyday my father makes improvements with their help. Also can t forget the amazing Dr.Zidek and NP Rebecca who not only took care of my father as their patient, but also made sure my family and I where in good hands as well. We felt so comfortable and relieved with their help and together as a team they are truly amazing and constantly making positive moves to improve my father. This facility cares like family here and everyone does the best they can do for their patients. As a medical professional myself I was pleased with this facility and their are a lot of great opportunities for my father here.

No baths were given unless we made them. she has bed sores, because they didnt help in turning on her side. She had ants in her bed, that hey finally made a decision 24 hours later to move her to another room, after shed been bitten all night. If you have any kind of compassion for human life, please go find another facility for your loved ones.

Looks can be deceiving. Great PT, OT, ST cant complain. A handful of nurses and cnas were very good. Tomorrow marks 4 yrs. Since my Mothers death. This facility should be shut down. They should never admit bedbound patients post stroke. They allowed her to develope a bed sores that never healed. I hope one day that each of them develope bedsores that never heal. Survivors of this place are walkie talkies. The only reason I gave a 1 star is due to the few good staff. I want the nursing home laws changed. The state needs to look at every residents wound during their visits. Mostly bedbound Its not the post care. Its prevention. THIS WAS AVOIDABLE. END OF STOREY. Theres no question about the care after. As Ive always said is Dr. Guidry should have never been consulted Period. Shes the best but anyone knows the best medicine is prevention and thats whete yall failed

Not a night went by where my mother didnt call me at home begging me to call them because they were either late with her pain medication or it was wrong or didnt come at all , and it sometimes didnt feel like pain medicationat all . I shouldnt have to make a 45 min drive to get a nurse to walk a pill 10 feet but I did so 3 times and one of those a nurse actually had the nerve to tell me it was 15 min past visiting hrs . My mother said it was a horror house , and the only piece of sunshine that treated my mother with respect was Mona Lisa ,( mom said she was wonderful). If this wasnt enough discharge rushed her out of the room and stuck my mother in the lobby w all her stuff on a cart and called me asking where I was without any previous notice of discharge .Apparently the room is more valuable than the patient here at Baywind.

My experience with the following staff at BayWind were absolutely wonderful. These ladies names are Eboney, Sequirtle, Priscilla, Jeanne, Jamie, and Tawana. There was one other night aide that was wonderful and kind to me, unfortunately, I cannot remember her name. However, all this being said, I finally had to move because of the inaction of other staff. My pain medication was not given to me per the doctors orders. It was the night staff that thought it was funny to withhold it from me. What they dont realize is that I could hear them laughing and talking about it in the hall. Every night I had to stress all night long begging for my paid medication. I was also rolled off into the floor hitting my head on the floor by a night aide. He just stood by and did not seem very concerned. My roommate also fell and while she was trying to call her daughter, the staff took her phone away and said they would call her daughter. From what I understand, they never called her daughter. They had xrays taken in the room but I dont think they ever gave her or her family the results. The food was beyond horrible. I will say the day staff was pretty good , probably because the owners were there during the day. I heard the night staff talking about how to avoid the cameras in the hall and I could also hear night staff hiding out in my room on their cell phones after my roommate moved out. The people named above and probably all of the day staff deserve a huge raise because of the devotion and their hard work. Except for one night person, and she knows who she is because I was always delighted when she was on duty, the rest of the night staff was substandard in my opinion.

She is understanding and fully equipped to help someone in all cases. She has helped me through numerous stays at Baywind with complete results each time. I want to commend her and Baywind for her help and knowledge. The next two people that I would like to commend are nurses Lisa and Beatrice. I had both of them on the night shift on hall 3. They are good in their profession. They are attentive and thorough to all of their patients. They both leave you with a feeling of safety and understanding in order to sleep through the night. There are others that need to be commended as well from Amanda as nursing Director to Maria who cleans the floors. Thanks to all.

Jamie, Sarah, and Doug went and helped my mother even when they were not her nurse. So grateful for those three nurses. There was a nurse one night that literally slept in her chair all night. There are many nurses at this awful place that have any compassion. The neglect on these poor people should be, and I m positive is against the law. Ppl call for help and sit in their own fesces for up to three or four hours plus. I could go on and on but if this is not enough, I don t know what is. What a shame and may the lord have mercy.

My mom was only there for 2 days after some complications from a knee replacement. The care at this facility is almost indescribable. She became dehydrated the second day, and had to be transported to the ER. While I was there that second night before visiting hours were over it took over 30 minutes to get an ice pack for her knee. I ended up going to the nurses station to get it, and was treated very poorly. The most shocking part was though that the nurse/nurses aide (not exactly sure of her title) threw the ice packs on the ground, stepped on them with her foot and proceeded to hand them to me. I would highly recommend NOT placing anyone at this facility.

I would give negative if I could. The staff are nice and helpful and probably doing their best, and it is clean, but this facility is horrifically managed. My grandmother had incidents of not getting medication at the right time or not getting medication at all. In the three weeks she was there, she never saw the presiding doctor ONCE, and the presiding doctor never contacted me either. I imagine the workers here are overworked and underpaid. The place is certainly understaffed - again, I blame the management, the workers are kind and doing their best. There is one nurse to cover a dozen patients at any time on weekdays, worse during weekends. In the event where I was not there to advocate for my grandmother and force them to move and respond, they wouldnt. Responses to call lights were abysmal (because they are understaffed), I set timers on my phone when visiting and one call light took TWENTY (20!!!) minutes for a response, average ten minutes, and that was WITH ME THERE WATCHING THEM, I cant imagine what its like for residents who have no one outside to help. There is no differentiation between an emergency call or a regular call - if she couldnt breathe or was having an emergency like a stroke shed be long dead. They took her off the therapy services schedule for no discernable reason at one point, meaning she went from making progress with standing and walking to being bedridden for days (the physical therapist was upset about this as well). When she was discharged, despite me specifically asking and requesting for her medications, I was told the facility she was transferring to had the medications and that they would not give me her medications, and as a result she now has NO MEDICATION, and Im certain they simply threw away her prescribed medications she did not finish (another management error). When calling to resolve the issues I was left on hold after being escalated to management for TWENTY MINUTES before they HUNG UP THE PHONE (I have the call recorded and will use it to file a complaint). When I called back they again hung up the phone (also recorded). The place is just generally like a prison. I wouldnt send an enemy here, much less a loved one. It is an undignified place for elderly people to slowly rot.
